Essential:

About the role:

As a central point of contact for support workers and participants, the Relationship Manager role is core to what makes Hearth different to other support providers.

Responsibilities will include but not limited to:

• Creating and continuously developing relationships between participants, their family, and support teams
• Maintaining relationships with support coordinators, house supervisors, and other relevant community and external stakeholders
• Lead and support the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their individual care plan goals
• Coordinating a weekly staff roster for each participant and proactively resolving rostering issues.
• Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan.
• Compliance with Hearth policies and procedures to ensure a safe working environment and detailed, timely incident reporting

• Minimum 2 years experience working as a case manager in community services, welfare support, disability support or social work (essential)
• A relevant qualification in community services, welfare support, disability support or social work (desirable)
• Proven experience delivering high quality client-centred service that is targeted and relevant to their NDIS goals
• A Professional level of administrative and business communication, including case noting and report writing
• Excellent time management and organisational skills
• Strong interpersonal skills and high levels of resilience
• Working knowledge of Microsoft office suite and employee rostering systems
• Own car and driver's license

About the role

Our Customer Success Managers build strong relationships with our customers through relational and strategic engagement, delivering results that have a true impact on retail communities. You will partner with key stakeholders and focus on ensuring their organisation is seeing the value and achieving real crime reduction outcomes from using the Auror platform.

Through running pilots and leading project rollouts, developing and delivering strategic engagement plans for our retail partners, you will identify ways in which customer and user engagement can increase, and champion relationships can strengthen. With the support of the Customer Success Specialists, you will run training and presentations to varying groups of stakeholders, users, and Police to help them realise the value.

Your Responsibilities

• Owning the customer success relationship and supporting commercial Account Management through partnership with our Retail Partnership Managers (Account and Sales managers)
• Working with our key champions to understand their priorities and how Auror can add value through ongoing strategic engagement at a market, regional, or user level
• Building proactive customer engagement plans to ensure our customers are reaching their user engagement goals
• Leading pilots & implementations with expert project management capability, focusing on our larger more complex retail partners
• Working with the Customer Success Specialists to surface platform wins and outcomes and sharing those learnings to key champions at Loss Prevention director-level
• Partnering with customers on Change & Process Management
• Analysing and utilising customer data and creating insights to drive better customer outcomes 
• Fostering collaboration across retail and law enforcement
• Collaborating with Product, both as a subject matter expert and a customer advocate
• Strategize on user engagement for a one:many approach through inbound support requests via in app messenger, phone, and email

To ace this role, you will have…

• Previous experience in Customer Success, Customer Care, Account Management roles preferably in the tech industry
• You are analytical and have the ability to turn data into insights to inform structured and strategic solutions
• Your strategic and big-picture mindset will see you using multiple data points to ensure that everything we do as a team contributes to Auror’s long-term goals 
• Have experience driving change management within business processes, systems and technology with a people-first focus
• Ability to guide our customers through rollouts and implementations, using your project management and people skills to make it seamless and successful
• Have the ability to see where our users are coming from, understand their day-to-day and be able to be empathic in their views
• Enjoy working as part of a team and build strong working relationships but can also be self-starting and able to work alone
• Have an interest in the tech industry and have knowledge of Auror’s brand and story
• Are happy to travel for regular customer meetings and events

• Salary range: This will depend on the level of experience. We have scoped this role as an IC3 with a hiring salary range of $85,000 - $110,000 AUD

The Role

We’re on the lookout for an experienced, reliable, dedicated, compassionate and empathetic Support Coordinator Team Leader who wants to make a difference by leading our Support Coordination Team and support our participants in reaching their aspirations and leading fulfilling lives.

What you’ll be doing:

As the Support Coordinator Team Leader at Incluso Services you will be responsible with engaging both internal and external stakeholders, from the first contact with participants, to onboarding and continue with ongoing service delivery. You will need to be able to manage multiple priorities, lead your team and deliver specialist assistance to participants that require a higher level of Support Coordination.

What you’ll bring:

• Exceptional customer engagement skills and experience, delivering high quality, individually tailored service.
• A solid understanding of the person-centered approach, the principles of the NDIS and the National Disability Service Standards, with experience working within the NDIS and/or disability sector.
• Efficiently allocate referrals to ensure prompt access to appropriate services for NDIS participants.
• Create, review and revise NDIS Service Agreements as needed.
• Collaborate with NDIS stakeholders to convert inquiries into referrals while maintaining exceptional client service.
• Work closely with operational team members and leaders to ensure a positive intake and service delivery process.
• Manage new and existing funding requests to ensure NDIS funds are available for service.
• Provide high quality and tailored advice to participants who are navigating the NDIS.
• Conduct face-to-face meetings with clients, including families and stakeholders, as necessary.
• Ensure robust case management activity with all activity entered into the system, ensuring all progress, plans, communications and interactions with participants and stakeholders are clearly documented.

What we would like you to have:

• Established networks within the NDIS and Northern Suburbs Community.
• Experience in a similar role or previous Level 3 Support Coordinator Experience.
• Authorised to work in Victoria.
• Valid Victorian Driver License
• An understanding of the NDIS, disability, and community services sector
• Relevant Qualifications: such as Bachelor of Psychology, Certificate IV in Community Services or studying towards relevant qualification will be highly regarded for these positions.
• Minimum 2-year industry experience
• Ability to actively navigate and adapt to change.
• Working with Children Check
• NDIS Worker Screening Check
• First Aid/CPR Certificate
