• Authorised to work in Victoria.
• Valid Victorian Driver License

Essential:

• Bachelor of Nursing
• Current Australian Health Practitioner Regulation Agency (AHPRA) registration
• Nursing experience within a residential aged care facility, preferably at least 3 years of aged care experience.
• Experience of working cooperatively with multi-disciplinary care teams
• Strong clinical assessment, documentation and practice skills
• Effective communication and interpersonal skills
• Proficient computer and information management skills, including ability to use Microsoft and clinical software systems.
• Experience in a similar clinical care role
• An understanding of the aged care accreditation process
• Working knowledge of quality improvement programs
• Current Driver’s License.

About The Role: The Clinical Care Coordinator plays a crucial role in coordinating person-centered clinical care for residents, adhering to established policies, procedures, and professional standards. Working in collaboration with the Care Manager, the primary purpose is to concentrate on individual resident needs. This involves ensuring the existence of a current and well-documented Care Plan that effectively addresses residents' needs. The Care Coordinator also contributes to the review and delivery of the Care Plan with the support of the multi-disciplinary care team.
