About the Role
As an Outcomes Lead at Community Connections, you will play a pivotal role in mentoring and guiding a team of Disability Support Workers to deliver high-quality, person-centered outcomes for the people we support. Your responsibilities include:
- Understanding, developing, and refining individual programs of support in line with a person’s NDIS goals
- Developing links to a person’s community that offer them greater opportunity for inclusion and the development of valued social roles
- Onboarding, supporting, supervising, and developing of assigned team members
- Managing transport logistics during service delivery
- Collaborating on service quality monitoring, planning, analysis, and reporting
- Working with the Leadership team to ensure smooth rostering and system updates
- Collaborating with peers to maintain accurate person-supported information and documentation
- Reviewing and approving Carelink entries in line with payroll and billing requirements
- Delivering hands-on disability support work as required
- Student placement supervision as required
About You
We're looking for someone driven to make an impact to the lives of the people we support and a genuine passion for building capability and supporting their team.
You will:
- Be a natural influencer, modelling a positive attitude, professional conduct and unwavering commitment to quality outcomes and service delivery for staff, individuals with disability and the community.
- Possess a deep understanding of the NDIS, its funding, and systems.
- Have experience in leading and inspiring small teams
- Have prior support worker experience including demonstrated understanding and knowledge of the tasks of a disability support worker
- Be committed to service excellence, human rights, and zero tolerance to violence, abuse, neglect, and exploitation.
- Have experience running a day service (desirable)
