Join our dedicated team as a Wellness Facilitator and play a key role in delivering exceptional aged care while supporting our wellness and reablement philosophy. Some of your responsibilities will be:
Engaging and enrolling new clients referred to Specialised Support Services as allocated
Using the appropriate assessment tools conduct comprehensive assessments, reassessments and evaluation of client care needs, providing clinical advice to clients, carers and other health care professionals
Providing ongoing support including referral and transition to other clinical and non-clinical services internally and externally for clients receiving Specialised Support Service
