Support clients in Gold Coast Region
The Case Manager will be required to complete initial intake assessments, develop and maintain care plans, perform holistic case management, monitor and review consumers under a framework of active partnerships with consumers and their family/carer. You are required to apply the principles of wellness and reablement and facilitation skills to achieve quality consumer outcomes. Responsibilities will include:
To build strong relationships with consumers and their family/carer and are active partners in developing an effective support plan for the consumer
Ensure packages are filled, waitlists are maintained, and budgets managed effectively under direction from the Case Manager
Ensure consumer care and case coordination is performed in accordance with best practise standards, legislation and AQ philosophy, policies and procedures
