Creating, updating, and publishing the roster on a fortnightly basis.

About the role: 

We are seeking a Community Case Manager to join our team and play a vital role in supporting elderly people in the community to live independently and with dignity in their own homes.

As a Community Case Manager, your primary responsibility will be to manage a portfolio of community-based elderly customers with Home Care Packages. You will assess their needs, develop personalised care plans, and manage budgets to ensure each customers' unique requirements are met.
