About the Role
An opportunity currently exists for a Grade 1 Social Worker to join our team at St Georges Hospital, Kew. To fulfil this role, it is essential that you have experience working in an inpatient hospital social work position. Experience in a sub-acute (GEM or Rehab) setting will be well regarded.
To perform this role, you will:
Use your skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans
Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes
Actively participate in quality improvement and research activities
Promote and develop positive relationships within the department,
