Job Description:
Beyond Ability Services, the registered NDIS service provider in Australia, is seeking a dedicated and compassionate Disabilities Services Officer to join our professional team. With years of experience in providing a wide range of disability and support services to NDIS participants, we are committed to delivering exceptional care to individuals with complex needs.
Key Responsibilities:
1. Client Assessment and Program Development:
o Assess clients' needs through interviews and evaluations to determine the nature and extent of their difficulties
o Plan, develop, and implement personalized educational, training, and support programs, including assistance with personal activities, daily tasks, and shared living arrangements
2. Progress Monitoring and Reporting:
o Regularly monitor and report on the progress of clients to ensure their needs are being met and they are receiving the necessary support to achieve their goals
o Maintain accurate records and documentation of client assessments, interventions, and outcomes
3. Referral Services and Community Liaison:
o Identify and refer clients to agencies that can provide additional support and assistance, such as health, welfare, housing, employment, and training services
o Liaise with community groups, welfare agencies, government bodies, and private businesses to address community issues and promote awareness of available resources and services provided by Beyond.
4. Family Support and Education:
o Provide support, education, and care for children and disabled persons in adult service units, group housing, and government institutions
o Work closely with families to ensure they have the resources and knowledge necessary to support their loved ones, including assistance with personal activities and household tasks
5. Funding and Reporting:
o Prepare submissions for funding and resources to support Beyond Ability Services' programs and initiatives, including Supported Independent Living (SIL) and Innov Community Participation
o Compile reports for government bodies and other agencies, ensuring compliance with relevant regulations and guidelines
6. Transportation and Travel Assistance:
o Coordinate safe and comfortable transportation for clients unable to use public transport, ensuring they have access to the support and services they need
o Plan and organize transport and travel needs, assisting clients in accessing funding through the NDIS
What we are looking for:
Skills & Experience:
1. Relevant qualifications in the health care sector desirable
2. At least three years of work experience in the industry
3. A comprehensive knowledge on age as well as disability related health issues
4. Excellent communication and interpersonal skills including demonstrated experience in interacting with a wide range of people.
5. A compassionate and empathetic approach to supporting individuals with disabilities
