This position is required to support CRCS' Commonwealth Home Care Packages Program in planning and delivering a range of services to ageing participants within the ACT and Capital Region, to support them to maintain capacity for independent living, and to support their carers. This position will also facilitate services for people who require in home support, individual social support, transport, group social support, allied health supports and other supports as required, including brokered services. Reporting to the Coordinator - Aged Care Admin Services, the Case Worker is a skilled communicator, well organised and has acute attention to detail with excellent timekeeping and case note accuracy.
