About the Role An exciting opportunity currently exists for a Social Worker to join the Health Independence Programs (HIP) team. The Social Worker will deliver high quality person-centred, goal-orientated and home and centre-based care within a supportive multidisciplinary team.
This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.
Your Contribution
Minimum of four (4) years? post-graduate experience, preferably in a rehabilitation or hospital setting
Eligible for Registration with Australian Association of Social Workers (AASW)
A demonstrated sound knowledge and application of social work theory, practice and principles in the areas of rehabilitation
