Integral Home Care: Disabilities Services Officer
Position: Full Time (38 hours weekly)
Job Description:
Work as part of a team to support people with disabilities living in their own homes.
Complete all required competencies appropriate to designated client.
Abide by workplace health and safety standards
Provide social support.
Follow Care Plans, Meal-time Management Plan, swallowing guidelines belonging to client.
Provide disability support with individuals and families.
Encourage positive and socially appropriate behavior, and effectively manage challenging behavior.
Familiarize with client Behavior Support Plan and follow the same.
Accommodate changes in line with principles of continuous improvement.
Complete online and Face to face training allocated on AUSMED.
Respond to crisis within specified guidelines and seek assistance to situations outside specified guidelines.
Maintain current knowledge, understanding of the organizational and departmental policies, procedures, and guidelines.
Deliver Person Centered Care – Liaise with participant and family to achieve required/set goals.
Maintain second yearly Police checks, mandatory NDIS workers screening, and working with children check.
Works in a range of services to aid and community access to people with intellectual, physical, social and emotional disabilities.
Responsible for meeting/interviewing clients and assessing the nature and extent of their needs.
Up to date reports kept on wellbeing and progress of clients.
Assist participants with personal care needs and undertake care plan for individuals.
Use communication and interpersonal skills to assist disabled clients.
Assist Social and Community Participation
Assist Medical Appointments and Follow-up Appointments
Daily Documentation of services provided and charting of daily activities
Skill Descriptors Maintained:
? Relevant Certificate in community worker
? Relevant work experience.
? Basic Life Support, and First Aid Training Certificate.
? Read and Sign of Care Plans, Meal-time Management Plans, and Behavior Support Plans-every six months.
? Police Check
? Excellent Communication skills (written and verbal),honest, reliable, and attention to detail.
? Appreciate existence of various disabilities, along with patience and relevant approach in professional caring of individuals.
? Provide assistance to Aged care/Disability participants to live independently at home.
Salary $75,000 p.a. + super
