Discharge Coordinator - Social Work Permanent Full Time Northern Beaches Hospital seeks a highly motivated person to join our friendly and hardworking Social Work team. The Social Work team offer a caring environment with opportunities for development for the successful candidate. You will be expected to manage multiple stakeholders (internal and external) from a range of disciplines and come into direct patient contact. As a Discharge Coordinator you will:
Collaborate closely with the social work team to secure Residential Aged Care Facility placements
Collaborate with Hospital Clinical Operations Manager to enhance patient flow and reduce length of stay in hospital
Lead and coordinate various health care teams to meet the ongoing NDIS requirements of in-patients
Assist with the administration and technical clinical tasks
Liaise with the family & multidisciplinary team
Liaise and complete relevant process/documentation for transfer of care
*Customer service is expected to be of high standard and this is reflected by positive patient experiences. Essential Criteria:
Previous experience in a front line customer/patient facing environment
Proactive, able to work within a team environment and take directions
Exceptional customer service, responding to patients and stakeholders in a timely, efficient, friendly and courteous manner
Exceptional attention to detail
Ability to prioritise work
Well-developed oral and written communication
Good standard of computer literacy
You will work well as a team member, whilst also being a self-starter, and able to manage multiple priorities. You will need a flexible to assist the team to suit department needs under the direction of the Social Work Manager.
