FRANKSTON HOSPITAL – SPECIAL SKILLS PLACEMENT – ULTRASOUND
Point-of-care ultrasound (POCUS) expertise is a vital skillset for the modern Emergency Physician to acquire during their registrar training years. However, it can be difficult to achieve ultrasound mastery through the standard pathways of ED & external rotations during ACEM training. The fast pace of work as a registrar in an Emergency Department leaves little time-out for trainees to systemically practice and consolidate their POCUS skills during shifts.
Our unique Special Skills Placement in Emergency Medicine Ultrasound facilitates a focussed and structured approach to POCUS knowledge and skill acquisition, through a variety of activities including practical and online teaching. We provide a rich learning experience, with a high level of supervision. At times, the supervision will be at the bedside by ED consultants and members of other departments, with the remainder of the supervision being through remoteimage review. All scans will be recorded in a logbook and recorded. Regular formal case reviews, supervisor meetings and journal clubs will be conducted.
Candidates will not be rostered to any clinical shifts – only scanning shifts – and will not be picking up their own patients during these shifts. During scanning shifts, candidates will be supporting senior and junior medical staff in the ED by performing appropriate scans and ultrasound-guided procedures.
The rotation is best-suited to an ED registrar who has completed more than 2 years of ACEM training, and who does not have concurrent competing ACEM exam priorities during or soon after the rotation – in order to maximise their focus on ultrasound skill acquisition. Furthermore, senior trainees will in general be preferenced over junior trainees – in particular, trainees who have completed their fellowship OSCE exam. However, any ACEM trainee may apply.
Our rotation was accredited by ACEM including for TS4 trainees in 2023.
The standard ultrasound rotation is 6 months duration to provide the best opportunity for skill uptake, particularly in advanced echocardiography. Requests for a 3 month rotation may be considered, depending on candidate and departmental circumstances. A 12-month contract will be offered in most circumstances, with the registrar being assigned Emergency Department placements for the remainder of the training year.

In 2024 we were accredited by ASUM for 7 CCPU units – E-FAST, FELS, RCE, Vascular Access, AAA, Lung, Proximal DVT. This means we can supervise you in attaining these units during your rotation, without needing to attend an external training course.

Learning alongside echocardiographers in the echocardiography lab.
Global health opportunities – such as teaching on a PEARLS-ED and PEARLS-Crit Care course overseas.
Practical and theoretical coaching and guidance directed towards the annual January sitting of the American Society of Echocardiography (ASE) National Board Exam (NBE) Critical Care Echo Exam (CCEeXAM); with ongoing mentoring thereafter aimed at obtaining the Fellowship of the American Society of Echocardiography (FASE).
Access to the Frankston ICU state-of-the-art ultrasound simulator, facilitated by coaching from expert instructors, and a certificate of completion from University of Melbourne. Course offerings include Focussed Cardiac Ultrasound, Advanced Cardiac Ultrasound, Periarrest, Lung Ultrasound and more.
Participation in research projects within the department, and guidance towards authorship of case reports.
Collaboration with, and supervision of elective medical students to be attached to the Special Skills Placement from 2025.
The advanced trainee has additional main roles as follows:
Proactively managing Patient Safety, Clinical Risk, Quality Improvement, Customer Relations, and Infection Control issues as they relate to patients within their care: including the specific notification of all Incidents or Near Misses relating to patients within their care.
Supporting the utilisation of both evidence-based medicine and shared decision-making approaches with their patients. Supporting the development of relevant research activities.
Supporting the ongoing delivery and development of ultrasound training programs for Registrars, Junior Medical Officers, Interns and Medical Students within the department and in conjunction with Supervisor of Special Skill Placement, Director Emergency Education and Directors of Emergency Medicine Training.
It is expected that trainees attended registrar teaching, including state-wide meetings.

Frankston is 42 km or less than 1 hour drive from the City centre and is the gateway to the Mornington Peninsula. This is both a popular holiday destination has beautiful beaches, calm waters in the bay, surf beaches, wineries, golf courses, but also has a vibrant community.
Rosebud is a smaller Community hospital nestled in the heart of the Peninsula, perfectly positioned directly opposite the beach. EMR, bed management and all clinical systems are fully integrated between the two departments.
Peninsula Health
It is a major teaching and referral centre with over 360 beds. The ED has access to a comprehensive range of acute medical, surgical, geriatric, psychiatric, obstetric, gynaecological and paediatric services on site. All the major imaging modalities are on site.
The $1.1 billion redevelopment of Frankston Hospital will deliver a new 12-storey clinical services tower and main entrance, 130 more beds, new spaces for mental health and oncology services and 15 new operating theatres. Construction is underway, with main works expected to be completed in 2025. A new dedicated Paediatric Emergency Department and Crisis Hub is being built as part of the redevelopment.

The Department of Emergency Medicine
The Emergency Services currently treats over 100,000 patients per annum between the Frankston and Rosebud campuses, with 25% paediatric presentations. Both departments have active Emergency Short Stay units with a total of 40 beds across both sites and a further 10 beds planned as part of the current Frankston Hospital redevelopment. The admission rate is about 40%, with a diverse and high acuity casemix.
There are currently 45 consultants working in the Emergency Department across both sites including 3 ACEM examiners.
Peninsula Health Emergency Medicine has a strong educational program and is accredited by ACEM as a Tier 1 training site at Frankston ED and is accredited for Training stage 4. In addition to ACEM trainees, the Emergency departments also regularly train candidates for the EMC and EMD.
