About the Role
The Social Work Renal program is looking for an experienced Grade 1 Social Worker to join the team to work across Casey, Clayton, Cranbourne, Dandenong, Moorabbin and the Kingston Centre. This is a full-time, permanent role primarily based at Monash Medical Centre. This role is predominately based at Casey, Dandenong and Cranbourne Dialysis Units but work across all sites will be required. Experience working with individuals with chronic illness and hospital social work experience is desirable. This clinical role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and development of patient centred care plans. It is a requirement of this role to participate in the Emergency Department and on-call rosters.

We are a busy and dynamic team assisting patients and families by providing a professional service for patients referred across acute medicine and outpatient clinics. Our Social Work service includes acute and specialty areas such as Intensive Care, Emergency Department and Women’s and Children’s services. The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support.

About You
To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:
You are a qualified and experienced Social Worker with experience providing a strong focus on patient centred care
Through your hard working and enthusiastic nature you have the ability to adapt to changing settings and shifts during the business week and weekends or public holidays
You are a dynamic Allied Health professional who is passionate about making a positive contribution to the health of your patients
You are highly motivated to perform at your best every day, demonstrating excellent communication and interpersonal skills
Your dedicated to safe best practice and quality care
An unwavering commitment to delivering patient centred care
Possess a positive attitude to your work, compassion and the ability to work collaboratively and effectively in a team
Motivation to be accountable for and manage specific portfolios in our relentless pursuit of excellence
Experience in a hospital or other health setting is desirable.
What you need
To be considered for this important role you will ideally possess the following:
An AASW accredited Bachelor degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers
Experience in a healthcare setting is desirable
Good organisational skills and a positive, enthusiastic attitude towards work
Experience in working with culturally and linguistically diverse communities
Able to work autonomously
A friendly, outgoing and reliable demeanour
Well-developed clinical assessment and complex discharge planning skills
A current Victorian drivers licence
