As a Support Worker, you will be managing a caseload of clients whom you will assist resolve and/or reduce barriers to stable self-care with low intensity community supports or access higher intensity/duration supports to achieve therapeutic outcomes.

Responsibilities of the role: 

• Provide psychosocial support to clients (16-65 years) to achieve recovery goals.
• Empower clients to manage their mental health, make informed decisions, and build confidence.
• Enhance coping skills, reduce self-harm, and support daily living activities.
• Strengthen social skills and networks, including family and community connections.
• Support clients in accessing education, vocational training, and employment.
• Encourage a healthy lifestyle and reduce harmful behaviors.

Who we are looking for 
We are looking for a flexible thinker with not only extensive experience, but also demonstrated skills and knowledge in Mental Health and the Social Model of Health to join our multidisciplinary Mental Health team. 

To be successful in this role, you will have: 

• You must have experience as a Case Worker in mental health.
• Understanding of mental services interventions for responding to clients with complexity of needs.
• Understanding of vulnerabilities which people with psychosocial disabilities experience. 
• Ability to collaborate & work in partnership with Eastern Health Case Managers, Team Coordinator, your clients, carers, family and EACH peer workers to assess, design & review client recovery plans.
• Ability to develop & maintain effective relationships within the team & with external stakeholders.

As a Support and Engagement Coordinator, you will manage a designated case load with a mix of individual clients and the Community and Management of SRS (Supported Residential Services) facilities. Services are delivered flexibly, both in the community and within the facility, and can be tailored to individual needs, either 1:1 or in groups. As a member of the SRS Outreach and Assistance team, you will play a pivotal role in uplifting residents and proprietors through innovative capacity-building techniques. Your responsibilities will include:

• Encouraging greater resident engagement in a variety of activities, both within their SRS and in the broader community.
• Supporting proprietors and SRS staff in honing their skills and capacities to better understand resident needs, improve support planning, and effectively manage challenging behaviours.
• Partnering with health and community support services to enhance accessibility for SRS residents and improve their responsiveness to proprietors and staff.

Who you are

• Tertiary qualification in a relevant health or human services discipline and/or demonstrated skills and experience in community engagement, assessment and support planning in a Community Sector role
• Knowledge and experience of the Homelessness Sector
• A good understanding about NDIS and My Aged Care packages 
• Working knowledge of local service systems and referral networks 
• Desire to use advocacy as a way of improving the life/circumstances of our clients
• Current state-based driver’s license
