About Us

The Clayton Adult Emergency Department provides tertiary emergency services for adult patients across Southeast Melbourne and surrounding regional areas. The Department is responsible for over 60,000 emergency department presentations annually. Once fully commissioned, the adult emergency department will have 75 treatment spaces, including six resuscitation cubicles, eight isolation rooms and a 20-bed adult short stay unit.

There is also a purpose-built Mental Health and Other Drugs (MHAOD) hub, which includes six dedicated cubicles, interview, and sensory rooms to provide an optimal environment to assess and manage patients presenting with behavioural health needs. The Adult Emergency department sees presentations requiring tertiary level care for Neurosurgery and Stroke services and also includes a diverse range of specialist support services onsite.

About You

You are dedicated, driven and a leader in Adult Emergency Nursing.

To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:

Demonstrated leadership and senior nursing experience within and Emergency Department. Associate Nurse Manager experience required.
Post graduate Qualification in Emergency Nursing or Leadership and Management (or working towards) is desirable

About The Role

This is an exciting and rare opportunity for a qualified and experienced Nurse, Division 1 to work within the Early in Life Mental Health Service inpatient unit.

We currently have part and full time ongoing roles working across a 24/7 rotating roster.

About You

You are an experienced Mental Health Clinician with previous experience within a youth / adolescent mental health setting. You also have previous experience within adolescent mental health nursing, and familiarity with milieu therapy. You also have excellent communication skills and are able to work well with young people, related services and their families within the multidisciplinary team setting. You will also work collaboratively with the various components of the ELMHS service, and provide leadership to the unit on a shift by shift basis, and add to the leadership group of Stepping Stones. About Stepping Stones Adolescent Inpatient Unit at Monash Children’s Hospital

The Monash Children’s Stepping Stones Adolescent In patient Unit is a 15 bed inpatient and five transition (Day) bed unit that offers care for adolescents aged 12 to 18 from broader ELMHS. The Adolescent Unit offers mental health Interventions to adolescents who present with acute and severe mental health problems, associated high risk factors and have difficulty maintaining their own safety and the safety of others.

There is a focus on those with traumatic injuries who have received plastic and orthopaedic surgery and there will be a variety of other clinical presentations. The Hand Therapy Team has a close relationship with the Plastics and Orthopaedics surgical teams and prides itself on delivering best care to maximise a patient’s return to function. Our commitment to service improvement also provides opportunities for involvement in research and implementation of evidence-based practice. These roles will have responsibilities across the full range of our hand therapy service, both in the acute trauma caseload and in the Adaptive Hand Therapy Clinic.

An exciting project initiated by the Department of Health being undertaken by Monash Health, the Adaptive Hand Therapy Clinic will provide evidence-based and effective intervention for patients with specified hand conditions that respond well to non-surgical treatment, thereby enabling increased numbers of people to be removed from surgical waiting lists. Clinical teaching is a core activity, involving staff supervision and active support of student placements. You will be involved in service development, working parties and clinical groups as needed, working with the Hand Therapy team and out-patient department to provide an excellent service and implementing service changes as required.

About the Role

We are presently looking for an experienced Grade 2 Social Worker for a permanent, full-time role. The Monash at Home, Aged and Rehabilitation Service is based at Kingston Centre in Cheltenham and at Cranbourne. The successful applicant will be expected to work at either site, or work across both sites.

About Us

The Monash at Home – Aged and Rehabilitation Care service, previously GEM at Home, launched in December 2020 and is a fundamental part of the broader Aged and Rehabilitation Division, providing safe and high-quality home-based subacute level care for individuals with complex medical, functional, and often cognitive issues, who require comprehensive, medically led multidisciplinary assessment and optimisation.

The service is seen as an alternative to hospital-based subacute care, designed to facilitate an earlier discharge from acute care and/or to avoid a subacute hospital stay entirely. Patients within the service receive at least daily visits from a member of the multidisciplinary team and access to 24-hour telephone support.?The model of care aims to provide short term, high quality, safe and effective patient centred care to individuals within the primary Monash Health catchment. The Program is entering an exciting phase of growth which will include, but not limited to, increasing the service clinical capacity and extending the geographical catchment.

About You

You are an accomplished Social Worker with exceptional clinical knowledge and assessment skills, and ability to work with a range of stakeholders. ?You have a genuine passion for delivering clinical care in the community setting and have demonstrated clinical leadership in this field. You have proven ability to deliver successful, timely outcomes?across service delivery, and you are an effective communicator who builds rapport rapidly. You can work autonomously, you are also a strong team player and can work efficiently in a team environment. You are passionate about achieving outstanding outcomes for our consumers. On a personal level, you can also demonstrate:

Expertise in delivering hospital level care in the home
Commitment to a supporting your manager and the team in a leadership and mentoring role
Proven time management, superior communication and influencing skills
critical thinking, analytical and problem-solving skills
What you need:

Bachelor of Social Work, Masters of Social Work Qualifying or recognised equivalent qualification
Accredited or eligible for ordinary membership of AASW, or written evidence of eligibility
Current Drivers licence without restrictions
“Completion or working towards a post Graduate qualification in relevant field” will be highly regarded
