We have an exciting new opportunity located in Barangaroo for Customer Assistants who would love to use their knowledge of wine to work in our new store - The Cellar. The Cellar will offer a dynamic and carefully curated range of relevant and interesting premium wines, craft & premium spirits and selected beers. You will have the opportunity to share your love for products with our customers.

What do we look for in a Care Worker? 

• Certicate III or above in Aged Care or Individual Support, Disability or Nursing.
• Experience of 6 months or more in a similar role including experience with personal care, manual handling, hoist training and medications.
• You care about assisting our aged community so they can live independently at home for longer. 
• You have a current P1 class or above Australian Drivers License and a registered vehicle – travel is part of the role.
• You don’t sweat the small stuff; sometimes your customer will be having a bad day or traffic delays you. You know not to take this personally and to shake it off before your next customer.
• You're available to work at least 3 days - Monday - Friday. No night or weekend work here.
• You're comfortable working independently and finding solutions to everyday problems, knowing when to ask your Manager for help.

In this Full-time role, based in Box Hill, you will be responsible for the end to end process of patient admission and ensure this is managed effectively and efficiently. You will be part of the Patient Services team and report to the Operations Manager.

You will make an impact by:

• Oversee and management the Health Fund Insurance Checks and ensuring patient billing is accurate and within agreed margins.
• Ensure budget is managed effectively and to agreed outcomes.
• Liaise with the finance team to resolve financial issues and ensure billing efficiency.

You will need to be a compassionate and a strategic thinker with a focus on positive patient experience. You will have a “can do” attitude and a solutions focused approach with a proven track record in meeting KPIs.

• Previous and recent hospital administration experience
• Evidence of a commitment to patient/customer service, clinical governance and quality improvement

Our redeveloped site delivers positive, meaningful, and sustainable impact on the health and wellbeing of our community, delivering much needed health care services and facilities in Melbourne's east.

The Stepped Care Coordinator is a pivotal role which has never been done before!

You will play a key role in the delivery of a new collaborative community mental health and AOD care model within one of four local hubs. The position has been designed specifically for the hubs and will manage the process of clients coming through the service (you will be the first contact with the client after referral from the Head to Health Phone Line or via other pathways and navigation to the appropriate service). 

Responsibilities include:   

• Direct client engagement and implementation of brief interventions to support clients throughout their care journey, ensuring smooth transitions between services and aftercare.
• Collaborating with internal and external stakeholders, including partner organizations.
• Assisting clients during their first interaction with hub services to ensure a welcoming and supportive entry.
• Overseeing collaborative care processes, such as care planning, transitions, and team reviews.
• Providing ongoing support for clients transitioning between programs, implementing aftercare models, and promoting sustained recovery.
• Building and maintaining key relationships, service pathways, and agreements with local hub partners, and collaborating with the local lived experience workforce to foster community-based responses.
• Monitoring care model outcomes and demand, coordinating collaborative responses to meet local needs.
