Coordinator - Stepped Care (Mental Health, AOD & Suicide Prevention program)

The Stepped Care Coordinator is a pivotal role which has never been done before!

You will play a key role in the delivery of a new collaborative community mental health and AOD care model within one of four local hubs. The position has been designed specifically for the hubs and will manage the process of clients coming through the service (you will be the first contact with the client after referral from the Head to Health Phone Line or via other pathways and navigation to the appropriate service). 

Responsibilities include:   

  • Direct client engagement and implementation of brief interventions to support clients throughout their care journey, ensuring smooth transitions between services and aftercare.
  • Collaborating with internal and external stakeholders, including partner organizations.
  • Assisting clients during their first interaction with hub services to ensure a welcoming and supportive entry.
  • Overseeing collaborative care processes, such as care planning, transitions, and team reviews.
  • Providing ongoing support for clients transitioning between programs, implementing aftercare models, and promoting sustained recovery.
  • Building and maintaining key relationships, service pathways, and agreements with local hub partners, and collaborating with the local lived experience workforce to foster community-based responses.
  • Monitoring care model outcomes and demand, coordinating collaborative responses to meet local needs.

The ideal candidate will have:

  • Strong leadership qualities.
  • Extensive experience in mental health and/or AOD settings, including knowledge of systems, legislation, and brief-intervention approaches.
  • Experience working independently and with care teams in community-based recovery.
  • Strong collaboration skills, with the ability to engage with various services and professionals.
  • In-depth knowledge of the mental health sector and community services for complex needs.
