Job Description: As a Case Manager at Assure Ability, you will play a crucial role in overseeing the delivery of services to individuals receiving Disability Support, along with their caregivers and families. Your primary responsibilities will include service planning, coordination, and collaboration with internal and external stakeholders to accomplish the objectives specified in participants care plans and service agreements.
Key Responsibilities:
· Strengthen and enhance participants capacity to coordinate supports, self-direct and manage supports and participate in the community, including providing participants with assistance to resolve problems or issues that arise understand their responsibilities under service agreement change or end a service agreement.
· Provide guidance and support to Support Workers to ensure the effective delivery of services.
· Liaise with various stakeholders including Assure Ability programs, partners, funding bodies, and government departments.
· Assist participants to prepare for their plan review by supporting them to assess whether they achieved their goals and got value for money for their plan identify solutions to problems experienced in implementing the plan consider new goals.
· Create, and design a service plan for those with complex and specialist needs including where collaboration with, and accountability to multiple services is required.
· Liaising and consulting with other Contribute to program evaluation, reporting, and compliance with audit requirements.
· Address complex barriers Coordinate responsive, individualized support to participants with complex needs, behaviours of concern, medical needs, or other high-risk circumstances.
· Assist in organizing events and promotional activities related to the Disability Support program.
· Maintain accurate data records and submit reports as necessary.
· Promote the program to NDIS service providers and potential participants.
· Maintain data collection systems as required by funding authorities and submit data and information reports as required.
· Working with Family and Children (under 18) to enhance their capability and smooth transition to social-economic environment for school-leavers with mental and Physical challenges.
· Working with the participant for Social Inclusion and transition from Jail/Custody to the community.
· Provide NDIA with reports on specific goals, outcomes, and success indicators within the agreed reporting frequency.
· Effective networking internally and externally to assist individuals achieve their desired personal outcomes.
Requirements:
· Bachelor/master’s degree in Community Service, Social Work, Psychology, Behavioural Science, or related field.
· Minimum of 6 months experience in a relevant role.
· Strong customer service skills with the ability to communicate effectively across various channels.
· Excellent attention to detail and organizational skills.
· Ability to work collaboratively in a team environment and adapt to change.
· Proficiency in time management and prioritization of tasks.
· Professional behaviour with the ability to work under pressure.
Position: Full-time
Salary: 70k - 80K + Super
