We are currently seeking an experienced and motivated Social Care Coordinator to join our team at Baw Baw views, in Moe VIC. This position is on a permanent full-time basis. We're looking for someone who has excellent communication skills, is highly organised, and has a knack for problem-solving.

As Social Care Coordinator, you'll be responsible for overseeing the day-to-day operational matters of the social care program at their facility. The Social Care Coordinator works in conjunction with the General Manager to provide and direct social care programs which are responsive, enriching and reflective of resident needs and requirements.

Essential:

• Certificate IV Qualification in Leisure and Lifestyle or similar discipline.
• Current National Police Certificate (or ability to acquire).
• Current Drivers License.
• Proficient computer systems and information management skills.
• First Aid Certificate.
• Excellent communication and interpersonal skills.
• A genuine interest for working in aged care.
• Well-developed customer service skills.
• The ability to lead and organise the social care team and volunteers.
• An ability to communicate with a diverse range of people.

Desirable:

• Experience in the provision of complimentary therapies and recreational services.
• Demonstrated experience in dementia care.
• Experience in residential aged care.
• Knowledge of and access to community groups and community support agencies.
• The successful candidate will ideally hold a Certificate IV in leisure and lifestyle or similar discipline.

The Positions:

We are seeking 15 Personal Care Assistants to join our team at our Alcheringa aged care home, located at 2-14 Boree Dr SWAN HILL VIC 3585.

Each candidate will be engaged on a full-time basis (38 hours per week); however, we would consider part-time (a minimum of 30 hours per week) if that were the candidate’s preference.

The proposed commencement date for each successful candidate would be approximately May/ July 2024.

The successful candidates will join our dynamic and supportive care team and will provide competent personal care, support and assistance to our residents under the direction and guidance of the senior clinical team.

Key duties and responsibilities include assisting residents with activities of daily living and lifestyle in accordance with an individual’s care plan, attending to care duties as allocated and/or delegated by the Registered or Enrolled Nurse, assisting with meal delivery and refreshments, and recording observations on relevant assessment charts and documenting an individual’s progress notes.

The successful candidates will have:

• A relevant qualification or the willingness to undertake on-the-job training.
• A genuine interest for working in aged care.
• An ability to communicate with a diverse range of people.
• A capacity to manage a diverse and demanding workload.
• The ability to work as part of a team.
• Flexibility for shift work, including nightshift and weekends.
• A current NDIS Worker Screening Check. Respect will assist each successful candidate in obtaining the required certification.
