We have an exciting new opportunity for an experienced Case Manager to join our friendly team at our Glen Iris office.

About the role:
As a central point of contact for support workers and participants, the Relationship Manager role is core to what makes Hearth different to other support providers.
Responsibilities will include but not limited to:
- Creating and continuously developing relationships between participants, their family, and support teams
- Maintaining relationships with support coordinators, house supervisors, and other relevant community and external stakeholders
- Lead and support the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their individual care plan goals
- Coordinating a weekly staff roster for each participant and proactively resolving rostering issues.
- Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan.
- Compliance with Hearth policies and procedures to ensure a safe working environment and detailed, timely incident reporting
- Minimum 2 years experience working as a Case Manager/Support Coordinator in community services, welfare support, disability support or social work (essential)
- A relevant qualification in community services, welfare support, disability support or social work (desirable)
- Proven experience delivering high quality client-centred service that is targeted and relevant to their NDIS goals
- A Professional level of administrative and business communication, including case noting and report writing
- Excellent time management and organisational skills
- Strong interpersonal skills and high levels of resilience
- Working knowledge of Microsoft office suite and employee rostering systems
- Own car and driver's license
