Job Description:
At Recovery Alliance Care Pty Ltd, we are seeking a passionate and dedicated Community Worker to join our team. In this role, you will be responsible for delivering vital community support services and facilitating community development initiatives. Your work will focus on addressing key community needs such as health, housing, employment, and social welfare, ensuring that individuals and families have access to the resources they need to improve their quality of life.

Key Responsibilities:
• Client Support and Needs Assessment: Conduct initial assessments of clients' social, emotional, financial, and welfare needs, ensuring that appropriate support is provided. Interview clients to understand the challenges they face and develop personalized support plans.
• Referral and Resource Coordination: Facilitate client referrals to relevant external services and agencies for specialized support (e.g., mental health services, housing support, financial assistance). Ensure timely follow-up to track clients’ progress and ensure continued access to services.
• Community Engagement: Facilitate clients to access the community and provide them with support as required. Attending appointments or social outings.
• Personal Care: Assistance with bathing, showering, brushing teeth, and getting dressed. Toileting, incontinence care, and menstrual care. Eating and drinking. Medication reminders Using aids, devices, and appliances.
• Monitoring and Reporting: Regularly monitor and evaluate the progress of clients, adjusting support plans as necessary. Prepare reports for internal use and external agencies, documenting client outcomes and any further resource requirements.
• Family and Youth Support: Provide direct support to families, children, and youth in need of assistance, particularly in overcoming social, emotional, and financial difficulties. Offer guidance and care in various community settings, including group housing and family support services.
• Supervision and Mentoring: Support individuals on probation or parole by providing guidance and monitoring their progress, ensuring they relate to appropriate services to aid their rehabilitation and reintegration into the community.

Required Qualifications and Experience:
• Relevant qualification at the minimum level of Diploma (AQF).
• At least 1 year of work experience in a community or welfare support role, demonstrating the ability to manage and coordinate programs and services effectively.
• Strong communication skills, with the ability to work collaboratively with clients, community members, and partner agencies.
• A genuine commitment to supporting vulnerable individuals and improving community wellbeing.

Desirable Skills:
• Experience in working with diverse community groups.
• Knowledge of local resources and services available within the community.
• Ability to manage multiple tasks effectively and meet deadlines.

Position Type: Full-time, Permanent (40 hours working per week).
Salary: $73,150 - $80,000 + Superannuation
