Family Support Worker - Aged Care

All Aged Care We are looking for a dedicated Family Support Worker to join our team, taking on a pivotal role in providing guidance and assistance to families, enhancing their social, emotional, and practical well-being. The ideal candidate will develop and deliver personalised support programs that cater to each family’s unique needs, monitoring progress and reporting to the Supervisor. Task & Responsibilities Assess clients' needs and design, develop, and implement educational, training, and support programs tailored to each family's situation. Conduct client interviews to assess the nature and extent of any difficulties, providing compassionate support and identifying solutions. Monitor and report on clients’ progress, adjusting programs as needed and keeping the Supervisor informed of developments. Refer clients to additional agencies for specialised support, ensuring they have access to a full range of resources. Assess community needs and resources in areas such as health, housing, employment, and welfare, identifying gaps and potential solutions. Liaise with community groups, welfare agencies, government bodies, and private businesses to address community issues and promote awareness of available resources and services. Provide support, education, and care for families, children, and people with disabilities within adult service units, group housing, and government institutions. Supervise offenders on probation and parole, offering guidance and support to help them reintegrate successfully. Assist young people with social, emotional, and financial challenges, promoting positive outcomes for their futures. Prepare funding and resource submissions, and compile reports for government bodies and other agencies. Qualification & Experience AQF Certificate III including at least two years of on-the-job training, or AQF Certificate IV  Minimum 12 months of experience in a similar role Enthusiasm for working with aged care individuals  A passion for helping and assisting families and their loved ones  High levels of verbal and written communication skills About Company All Aged Care is a dedicated provider of aged care and disability support services.
