Details: A career opportunity has arisen in ACT Corrective Services for a suitably qualified person with demonstrated clinical experience, personal integrity, self-confidence and exceptional communication skills for a Disability and Complex Care Coordinator (HP3) within the Offender Reintegration team.
The Disability and Complex Care Coordinator provides specialised clinical care coordination for detainees with disability and complex needs. The Disability and Complex Care Coordinator is responsible for ensuring individual needs are assessed and implement clinical interventions, engage in stakeholder engagement, and undertake functional assessments for complex needs.
