Position Title: Full time Aged or Disabled Carer
Salary: $66,000 – 76,000 per annum (including remote location allowance) plus superannuation and benefits
Tasks include:
PURPOSE AND OBJECTIVE OF ROLE:
The major objective of this role is to provide nourishing meals in a safe and clean environment for our Elders and teams.

KEY ACCOUNTABILITIES / POSITION ACTIVITIES / INHERENT REQUIREMENTS

Key Responsibilities:
• Daily living experience, which includes personal care and hygiene, showing, assistance
at mealtimes, and general mobility.
• By direction support the health and wellbeing needs of the Elders in line with their
care plans.
• Working with other Management, staff, Elders, their families, and carers to improve
health and wellbeing outcomes, and lifestyle for Elders.
• Working collaborative with management, staff and significant others to promote the
practice of quality, person centred care.
• The position regularly makes decisions in regard to client service plan and priorities.
• Written and/or personal communication skills are required as these roles have
responsibilities across a broad range of responsibilities.
• Understanding technology and how best to use would assist.
• High quality care is delivered in accordance with relevant frameworks and guidelines.
• Improved health outcomes for elders.
• Effective collaboration between staff and external stakeholders to delivery client centre services.
• Assist aged and disabled individuals with daily activities.
• Perform light housekeeping duties, such as vacuuming and cleaning.
• Aid clients with personal hygiene and dressing needs.
• Offer companionship, emotional support, and friendship.
• Support clients with mobility and prepare their meals.
• Organize and facilitate social activities for clients.

Role-Specific Duties:
• Coordinate the intake process for new clients.
• Develop, implement, and review individualized client service plans.
• Manage client services in line with NATSICFAC, CHSP, and HCP guidelines to ensure positive outcomes.
• Assist with referrals from Regional Assessment Services and ACAT in a timely manner.
• Foster strong collaborative relationships with ACS staff, programs, and external agencies for integrated service delivery.
• Actively participate in the Duty Roster.
• Advocate for increased services based on unmet needs or health changes.
• Provide support for incident and injury management for Community Service Workers and contractors.

Team-Based Responsibilities:
• Support Community Service Workers, local Coordinators, and contractors in delivering culturally sensitive care.
• Regularly engage in team planning and meetings.
• Develop and maintain professional and culturally safe relationships both internally and externally.
• Participate in continuous quality improvement activities.
• Take part in working groups, committees, networks, promotional events, and other organizational activities.
• Promote and represent ACS positively to staff, clients, stakeholders, and the community.

QUALIFICATIONS / EXPERIENCE

• Tertiary qualifications in Aged Care of Individual Support or equivalent (Certificate III or higher)
• Current first aid certificate
• Experienced in aged care / home care
• Working with Aboriginal people.
• Providing safe and effective care.
• Supporting delivery of care plans in line with principles of self-determination.
• Demonstrated ability to effectively advocate, liaise, network, and negotiate with a range of stakeholders.

COMPETENCIES

• Knowledge of aged care.
• Well-developed communication skills, and the ability to communicate respectfully and professionally, including communication of care requirements and processes.
• Ability to provide whole of person care in collaboration with other staff.
• Ability to actively engage clients.
• Strong organisational abilities, including effective time management and ability to prioritise competing work tasks in a dynamic office-based environment.
• Behaviour in manner consistent with relevant legislation, organisational policy, and procedures.
• The position regularly makes decisions in regard to client service plan and priorities.
• Written and/or personal communication skills are required as these roles have responsibilities across a broad range of responsibilities.
