About the role
We are seeking an Aboriginal Care Navigator for the Medicare Mental Health Centre (MMHC) in Wagga Wagga. This role works collaboratively with Aboriginal consumers to connect them with services and supports in the community, facilitating person-centred and integrated care.
This position reports to the Service Manager and offers the opportunity to:
Provide practical support through information sharing and education.
Identify and address barriers to access, facilitating warm referrals to ensure consumers receive the right service
