Position Summary:
9D Care is seeking a dedicated and compassionate Community Worker to join our team. The successful candidate will be responsible for evaluating clients' needs, designing and implementing support programs, and collaborating with various community organizations to provide education, training, and assistance to individuals. This role involves helping clients navigate social, emotional, and financial challenges, as well as providing care to individuals with disabilities, children, and those under probation or parole. The ideal candidate will have a strong understanding of community resources and a passion for making a positive impact on people's lives.
Key Responsibilities:
Evaluate clients' needs through interviews and assessments to understand the scope and severity of their challenges.
Design, create, and implement programs for education, training, and support tailored to the specific needs of clients.
Track client progress, prepare reports, and provide feedback on the effectiveness of support programs.
Direct clients to appropriate agencies and services for further assistance, including health, welfare, housing, and employment services.
Evaluate community needs and available resources, identifying areas where additional support is required.
Collaborate with community organizations, welfare services, government agencies, and private enterprises to address community concerns and increase awareness of available resources.
Offer support and education to families, as well as provide care to children and individuals with disabilities in residential units, group homes, and government facilities.
Oversee and support individuals on probation and parole, helping them reintegrate into the community.
Assist young people in navigating social, emotional, and financial challenges, providing guidance and resources.
Draft proposals for funding and resources, and prepare reports for government and other agencies to secure necessary support.
Qualifications:
Australian Citizens and Permanent Residents are encouraged to apply.
Minimum Diploma in Community Services, Social Work, or a related field.
2-3 years of relevant experience in community work, support services, or a similar role.
Knowledge of community resources, social services, and support programs.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and as part of a team.
Compassionate, patient, and committed to helping individuals in need.
Current First Aid and CPR Certification (preferred).
Valid driver's license and willingness to travel to various locations as needed.
Successful completion of a background check.
