Develop, implement, and review care plans that meet clients' needs using a Consumer Directed Care (CDC) approach
Organize and monitor brokered services with external providers
Manage HCP budgets, ensuring alignment with clients? goals and current government guidelines
Complete required documentation for new referrals and manage ongoing care for existing clients
Maintain accurate client records, budgets, and expenditure reports
About you
Qualifications in Nursing, Social Work, Allied Health, or a related field (preferred but not essential)
