As Coordinator you will be responsible for leading the Healthy Hubs Connect (HHC) model of care from the Coffs Harbour office.
Your duties will include:
- Engaging directly with clients in a brief capacity, as well as overseeing their care journey both internally and externally to the relevant hub.
- Implementing and overseeing processes at the hub to promote collaborative care for service users.
- Co-implementing a model of brief intervention after-care for targeted HHC customers.
- Monitoring outcomes and demand associated with the HHC model of care, and collaborating with HHC partners to coordinate impactful collaborative responses to local needs.
- Working in partnership with the local HHC lived experience workforce to build communities-based responses at the Hub.
