The Role:
The Allied Health Lead for Complex Care Coordination is a new role providing operational management of a small team of Complex Care Coordinators based at Launceston General Hospital. The role works closely with Nursing, Medical, Allied Health, and senior leadership across Hospitals & Primary Care North, in high-level problem resolution for complex and challenging patient issues, such as identifying hospital care pathways and overcoming barriers to sustainable discharge into the community.
