Position Overview:
We are seeking a highly motivated and experienced Disability Service Officer to join our team in Perth. The ideal candidate will have 2-3 years of experience in the disability services sector and possess the relevant qualifications to provide outstanding support to our clients.
Key Responsibilities:
• Develop and implement individualized support plans for clients in collaboration with their families and other stakeholders.
• Coordinate and deliver high-quality support services to individuals with disabilities, ensuring their needs and goals are met.
• Advocate for clients’ rights and empower them to achieve greater independence and inclusion within the community.
• Maintain accurate and confidential records of clients’ progress and support activities.
• Collaborate with other team members and external service providers to ensure comprehensive service delivery.
• Stay informed about sector developments, policies, and best practices to continually improve service quality.
Qualifications and Experience:
• A minimum of 2-3 years of experience working in the disability services sector.
• Relevant qualifications in Disability Services, Community Services, Social Work, or a related field.
• Strong understanding of person-centered practices and principles.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a diverse team.
