About the Role The Young Adult Complex Disability service (YACDS) is a state-wide multidisciplinary service for adults aged under 40 with a history of developmental disability, such as cerebral palsy, or other medical conditions resulting in chronic physical disability. The key aim of the YACDS is to help these young people and their family transition from paediatric services to adult healthcare services, and improve their health outcomes.
Delivery of accountable client focused, high quality referral triage, assessment, care planning, referral management, reviews, counselling and carer support within caseload
Collaborative work with the multidisciplinary team members, referring professionals, community professionals and other key stakeholders within a flexible, multidisciplinary structure
