Key Responsibilities:
Assess client needs and coordinate support programs on a case-by-case basis.
Onboard clients in collaboration with their chosen representatives, respecting individual choices.
Establish effective and streamlined rosters of client services based on individual preferences.
Work collaboratively with support workers and contractors to ensure clients receive quality service in line with their program goals and expectations.
Monitor overall program costs and ensure they are within budget parameters.
Monitor and report on client progress.
Maintain regular contact to build strong relationships with clients, carers, families, and other stakeholders involved.
Plan and implement socially inclusive programs and projects for the betterment of clients.
Maintain accurate records and documentation in accordance with organizational and legislative requirements.
Facilitate and promote the participation of service users in community activities and events.
Requirements:
Relevant tertiary qualifications in social work, aged care, or nursing.
Experience in providing services to NDIS clients as a Care Coordinator.
Familiarity with the NDIS Code of Conduct and guidelines.
Excellent record-keeping skills and the ability to monitor and report on client progress.
