Care Centrix is a Registered NDIS Service Provider working since 2019. We at Care Centrix work with a passion to provide the highest quality in disability and aged care support services.

We are seeking to employ multiple professionals for the role of Disabilities Service Officers based in Sydney NSW with renumeration of $74,000 to $80,000 per annum.

Candidates should hold a diploma or higher qualification, and 2 years of experience is desirable. This role is open to Australian citizens and permanent residents. We can consider visa sponsorship for suitable foreign professions.

• Provide personalised assistance to clients in daily activities, such as personal care, mobility, and communication.
• Act as an advocate for individuals with disabilities, helping them access necessary services and resources.
• Conduct assessments to identify the needs of clients and develop tailored support plans
• Regularly evaluate the progress of clients towards their goals and adjust support plans as needed.
• Referring clients to NDIS and NGOs for seeking further assistance on specific and complex client matters
• Preparing and forwarding proposals to NDIS for funding and resources based on client requirements
• Maintain clear communication with clients, families, and other professionals involved in the client's care.
• Work collaboratively with other professionals, such as social workers, therapists, and educators, to ensure comprehensive support for individuals
• Maintain accurate client records in compliance with NDIS and company guidelines for assessments, support plans, and progress notes.
• Respond to incidents within specified guidelines such disabilities services manual, code of conduct, disability policies and procedures, service duty rosters, and seeking assistance from certified / NDIS approved professionals as required
• Encourage participation in community activities and programs to promote social inclusion.
• Stay informed about best practices in disability support and participate in ongoing training opportunities.
