About the position
The Aboriginal Care Navigator, Medicare Mental Health Centre Wagga Wagga, works collaboratively with consumers to connect with services and supports in the community, facilitating person-centred and integrated care.
This position delivers practical support through information sharing and education, identifying and addressing barriers to access and facilitating warm referrals, ensuring consumers are linked to the right service at the right time. This position also works collaboratively with the other members of the Medicare Mental Health Centre Wagga Wagga team and external service providers
