About the opportunity
Care Coordination: Oversee and coordinate the delivery of individualized care plans for residents.
Assessment: Conduct comprehensive assessments of residents' physical, emotional, and social needs.
Team Collaboration: Collaborate with multidisciplinary teams, including nurses, physicians, and social workers, to ensure holistic care.
Compliance: Ensure adherence to regulatory standards and best practices in aged care.
Family Engagement: Communicate regularly with residents and their families to address concerns and provide updates on care.
Quality Improvement: Implement quality assurance programs and monitor care outcomes to enhance service delivery.
Staff Training: Provide training and support to staff on care protocols and documentation practices.
Crisis Management: Respond to emergencies and implement appropriate interventions as needed.
Skills and Qualifications
Clinical Expertise: Strong knowledge of geriatric care principles and practices.
Communication Skills: Excellent verbal and written communication skills for effective interaction with residents, families, and staff.
Organizational Skills: Ability to manage multiple priorities and maintain accurate records.
Problem-Solving: Strong critical thinking skills to address complex care issues.
Empathy and Compassion: Demonstrated ability to provide compassionate care and support to residents.
To thrive in this role, you will have:
? Excellent clinical skill base and knowledge of legislation within Aged Care.
? Experience in leading and supervising teams of people and liaising with families.
?Excellent time management, organisational and communication skills, with the ability to adapt and problem solve.
?A compassionate, flexible and positive approach.
? The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
You will also be required to provide evidence of, or in the process of obtaining:
? Recognised tertiary qualification and current registration as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA).
?National Disability Insurance Scheme (NDIS) Clearance.
? Current COVID-19 certificate, proof of current influenza vaccination and other vaccination status consistent with public health orders.
