Bullinah Aboriginal Health Service (BAHS) is an Aboriginal Community Controlled Health Service situated in Ballina, with outreach services in Cabbage Tree Island community at Wardell. Bullinah provides a full range of clinical support services and early intervention to the Community Members from Ballina and surrounding areas . The Aboriginal Health Practitioner (AHP) plays a key role in supporting our vision through direct service provision and team member support. The AHP will provide Health Care services
within Bullinah?s model and assist our Aboriginal Health Worker, nurses, medical staff and Reception teams.
Duties
1. Provide Pre-GP client screening, clinical care, and follow-up services to Bullinah Clients
2. Assist in the provision of Adult and Child Health Assessments
3. Assist with the development and implementation of Chronic Disease Care plans and team care
arrangements
4. Assist in the management of client recalls and notification systems
