About the Role:
South West Care Services Pty Ltd is a registered NDIS Service Provider. We specialise in providing in-home Care and Community Access to our Aged and disability participants throughout Central Sydney and Greater Sydney.
As a Disabilities Services Officer
You will work with empathy to complete a variety of domestic, social, respite, and personal care services to ensure a quality life with dignity, respect, and social inclusion.

Duties and Responsibilities:
• Comprehensive knowledge of the NDIS support items under the NDIS Guidelines.
• Assess, develop, and implement client support plans, ensuring compliance with the South West Care policies and procedures.
• Document all client’s responses to direct care/assistance given, include progress notes in accordance with the support plan and progress against NDIS Goals.
• Observe clients physical and psychological status and responses during interactions within scope of knowledge and report changes to the Service Manager.
• Working one-on-one or in a group setting supporting clients to solve social and emotional problems and make their own decisions to overcome barriers and strive towards achieving their goals.
• Communicate with clients and their stakeholders effectively and maintain positive working relationships.
• Assist in the delivery direct care activities, such as personal care, including showering / bathing; domestic duties, including vacuuming, mopping, and cleaning of living area, kitchen, and bathroom; community access, including transportation, meal preparation, wound dressing
• Advocate on behalf of clients to access services within the community.
• Supporting our clients’ families by providing information and care.
• Complete environment risk assessment of client’s residence and promptly report and document all hazards, potential hazards or unsafe actions. Preparing submissions for fundraising and reports to government bodies and other agencies.
• Undertake ongoing professional development as required and participate in the continuous quality improvement program.
In addition to the above responsibilities, you must also perform any other reasonable duties as directed occasionally.
Role Requirement:
• Qualification in Individual Support, Disability, Community Services or Aged Care.
• Current NSW driver’s licence
• Current First Aid Certificate
• Current Working with Children Check (or willing to obtain)
• Police Check (or willing to obtain)
• NDIS Workers Check
• Salary: $70-80k per year plus superannuation.
Skills and Qualifications:
• AQF Associate Degree, Advanced Diploma or Diploma in a highly relevant field
• At least 2-3 years of experience in a relevant position.
