Allambi Care are currently recruiting for the position of Family Preservation Caseworker, within the Outreach Youth Hope Program based in Charlestown.
Position Objective
The overall role of the Family Preservation Caseworke r is to work and support families and young people who would otherwise enter OOHC and leave their home, linking families into community supports where needed, and providing them with a framework to maintain the connection developed while connected to our service.
Primary duties and responsibilities;
Collaborate with the Manager on the intake and induction of new families
Follow Allambi risk assessment procedure when visiting and meeting families for the first time, and ongoing
Have regular case planning meetings with the family and young person ensuring to use a collaborative approach to any goals.
Review and assess progress of case plan goals regularly with the Manager and discuss any barriers to
