My Gravity Pty Ltd requires a committed Disabilities Service Officer who can work long-term with the company and independently manage the responsibilities of the role. The officer will be instrumental in upholding the company’s commitment to compassion and excellence in disability care.
KEY RESPONSIBILITIES:
Support and Care for Disabled Persons: Provide compassionate and professional care to individuals with disabilities in various settings, including adult service units, group homes, and other institutions. Ensure the well-being and comfort of clients, fostering a nurturing and respectful environment.
Client Assessment and Program Development: Conduct interviews with clients and their families to assess their needs and the nature and extent of their difficulties. Develop, plan, and implement personalized educational, training, and support programs based on individual needs, preferences, and goals.
Monitoring and Reporting: Continuously monitor the progress of clients, providing regular updates and detailed reports on their development. Make adjustments to care plans and support programs as necessary to ensure optimal outcomes for each client.
Collaboration with Healthcare Providers and Families: Work closely with clients, their families, and healthcare providers to create seamless and effective care plans. Ensure that all involved parties are informed and aligned in meeting the client’s needs.
Education and Advocacy: Provide education and training to families, caregivers, and clients to promote independence, improve quality of life, and ensure clients receive the highest standard of care.
REQUIREMENTS:
Australian Citizens and Permanent Residents are encouraged to apply.
Minimum of a Diploma in a relevant field.
2-3 years of experience in providing care for disabled individuals or similar roles.
Strong interpersonal and communication skills to work effectively with clients, families, and healthcare providers.
Ability to assess clients' needs and develop personalized care and support programs.
Competence in monitoring client progress and making necessary adjustments to care plans.
Capacity for self-management and taking initiative in managing the affairs of the company.
Experience with reporting on client development and maintaining accurate records.
A compassionate and respectful attitude toward individuals with disabilities.
Ability to work collaboratively to ensure a nurturing and supportive environment for clients.
Adherence to healthcare and disability support standards and regulations.
Commitment to maintaining the privacy and dignity of clients.
