About the Role You are a Community Mental Health Nurse who will bring experience in providing support to consumers from an intensive home and community based, Recovery orientated service.

Implement the Strengths Model to empower individuals in their recovery journey, emphasising their capabilities and resilience.
Foster strong therapeutic alliances with consumers and actively involve them, along with their family/carers/supporters, in comprehensive care planning and evaluation processes.
Conduct thorough assessments, deliver targeted interventions, and provide treatment for individuals with intricate mental health needs, aligning practices with local policies and strategic directions.
Contribute significantly to primary, secondary, and tertiary consultation services, actively engaging in community development, education, and establishing effective liaisons with other supportive agencies.
