About the role
This is a permanent full-time role which is based in Emerald on Gayiri country land (part-time may be considered for the right candidate).
As a Clinical Care Coordinator, youll work alongside the Clinical Lead and Operations Manager to deliver essential services across the Country to Coast Queensland (CCQ) region. You will be supporting individuals with severe mental health conditions by conducting thorough clinical assessments and developing tailored treatment plans. You will deliver evidence-based psychological interventions, manage a clinical caseload, and conduct risk assessments with detailed management plans.
Other responsibilities may include:
Engage in data collection and contribute to monitoring and evaluation
Collaborate with multidisciplinary teams and external stakeholders
