In this role, you will work alongside a Health Professional Level 3 Social Worker to provide comprehensive coverage for various services. These services include outpatient rehabilitation clinics at the Tertiary Hospital, the ACT Motor Neurone Disease Service, and the Dementia Rehab Service. Your work will be integral to the Day and Ambulatory Rehabilitation Service interprofessional team, collaborating with other health professionals to ensure seamless and effective care for our clients. Key Responsibilities: - Provide assessment and therapeutic interventions for a diverse range of client populations throughout their healthcare journey. - Address psychosocial issues that affect health circumstances, including ante-natal care, newly acquired and chronic health conditions, medical and surgical treatments, rehabilitation, and aging. - Work closely with an interprofessional team to deliver high-quality, client-centered care. - Engage in regular Clinical Supervision and offer Field Education to Social Work students. - Contribute to the continuous improvement of Day and Ambulatory Rehabilitation services and participate in potential rotations within other Social Work areas in RACS.
