Purpose of the Position

Abilifly Support Services is a registered National Disability Insurance Scheme (NDIS) provider with office in Mount Isa. We specialise in providing in-home support and community support/access to people with a disability, aged, children and young adults. We are seeking people who are physically fit, active, energetic, enthusiastic, and professional individuals and that have a solid experience in the community nursing and/or nurses in the disability sector to join our team in Mount Isa on a full-time permanent basis.
You will be reporting to the Service Delivery Officer/Manager, you will be required to support participants to reach their individual goals through meaningful and fun activities in their home and out in the community.

As a Nursing Support Worker you will:

Work in accordance with the medication administration policy, work health and safety procedures
and client right’s policies including privacy and confidentiality
Assisting with rehabilitation exercises, basic treatment and delivering medications
In conjunction with other members of staff assist in developing, implementing and evaluating
training and behaviour programs to attain identified individual goals.
Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes
Assisting people to perform important daily activities
Facilitating daily personal hygiene and dressing tasks
Observing and reporting changes in patients’ condition, and reporting complaints about care
Providing emotional support and friendship Support clients to participate in age-appropriate
activities and tasks including employment, in home support and community access.
Provide direct care assistance to each client in accordance with their service plan and as directed by
the Manager.
Monitor the safety and well-being of the clients and report any concerns to the Manager.
Ensure completion of appropriate documentation.
Relevant information on support provided is to be recorded in client notes and communication book
as required.
Work with clients, family/ carers, relevant case managers and other professionals to support the
client’s individual goals.
Assume the role of key worker for designated clients, including the preparation of a monthly report
on client progress.
Some travel may be required to clients who are outside of the Mount Isa region.

Essential Criteria:

Physically fit and active
The ability to develop a positive relationship with the participant
Excellent communication skills
The ability to work with children with disabilities
Able to work in a fast paced environment
Ability to acquire NDIS Worker Screening Check
Ability to acquire a Working With Children’s Check
Covid-19 Vaccination Certificate
First Aid Certificate is desirable
Cert II or III in Nursing (or equivalent) or other related fields is desirable
