The Client

Emerald Care Services is a registered NDIS provider that focuses on offering a wide range of holistic quality well-being and lifestyle services.

They are seeking passionate and qualified Disabilities Services O?cers to join their team in Melbourne, Victoria.



The Role

This is for multiple positions on a full-time basis with an annual salary set between $75,000 to $80,000 plus superannuation.



Responsibilities

Conduct comprehensive assessments of clients' needs to tailor support services accordingly.

Plan, develop, and implement educational, training, and support programs that promote clients' well-being and lifestyle improvement.

Interview clients to understand the nature and extent of their difficulties and develop appropriate intervention strategies.

Monitor and regularly report on the progress of clients, adjusting support programs as needed to ensure optimal outcomes with?focus on transparent communication around client supports.

Work in line with NDIS Code of Conduct and have a thorough understanding of what Choice and Control is for a clients and how to best uphold these rights, and have a moderate understand of the NDIS quality indicators and NDIS price guide.

Assess and refer clients to additional agencies and services that can provide further assistance and support.

Collaborate with community groups, welfare agencies, government bodies, and private businesses to address community issues and promote resource awareness.

Offer education and care for disabled individuals in various settings, including in shared and single-living dwellings to learn the life skills they need to build their own confidence in a home.

Provide education and support to families of individuals with disabilities, ensuring they have the necessary resources and information.

Prepare and submit funding and resource requests to government bodies and agencies to secure necessary support for programs.

Promote awareness of available community resources and services to clients and their families.

Advocate on behalf of clients to ensure they receive the services and support they need to improve their quality of life.



Requirements

In order to be shortlisted for this position, the right candidate needs to have:

Must hold a minimum Certificate III in Individual Support (Disability)

A minimum of 2 years of relevant experience

At least 2 years of experience and knowledge of disability, mental health and/or positive behaviour support

At least 2 years of experience working in the area of complex behaviour support with people

Willing to learn new skills and adapt your approach to meet the individual needs of the person you support.

Demonstrated ability to work within a team and independently.

Demonstrated ability to support and mentor staff under supervision.

Strong interpersonal and communication skills.

Effective prioritising, problem solving, time management and decision making skills.

Demonstrated ability to assist with daily programs and organising activities/excursions.

Effective communication skills with clients, families, and healthcare professionals.

Cultural sensitivity and respect for diverse backgrounds.

Documentation skills for keeping client care records.

Be able to handle pressure in a professional manner

Have positive past employment work references
