What you’ll be doing
We’re looking for an experienced non-clinical or clinical Case Manager, to play a crucial role in ensuring the delivery of high-quality care to clients participating in our Short-Term Restorative Care (STRC) program. The STRC program is an innovative and flexible care initiative that provides early intervention to optimise the functioning and independence of older people, aiming to reverse and/or slow functional decline.

As Non-Clinical Case Manager your duties will include:

  • Coordinating wellness and reablement services, social support, community services, multidisciplinary meetings, and administrative tasks in line with assessed needs and with support and oversight of appropriately qualified health professionals.
  • Undertaking customer assessments, and care planning with support and oversight of appropriately qualified health professionals.
  • Liaising with other agencies, health care professionals and specialists.
  • Co-ordinating the delivery of services within customer’s individualised plan and budget.
  • Working collaboratively with the customer and their support network to monitor and review the individual plan, ensuring supports remain and within the agreed plan.

As Clinical Case Manager your duties will include:

  • Overseeing wellness, reablement, and social support services based on assessed needs, including community services, multidisciplinary meetings, and administrative tasks.
  • Conducting initial and ongoing comprehensive assessments, create care plans, and support customers by monitoring health and clinical needs.
  • Providing guidance and liaise with other agencies and healthcare professionals.
  • Ensuring the delivery of services aligns with the customer’s individualised plan and budget.
  • Collaborating with customers and their support networks to regularly review and adjust care plans to maintain independence and meet program guidelines.

Who we’re looking for
Non-Clinical Case Manager

  • The ideal candidate for this role should have extensive experience in STRC or HCP delivery, demonstrating advanced care planning and case management skills.
  • Possess the ability to assess service provision to ensure high-quality and effective care for older adults, showing a high level of sensitivity towards issues impacting this demographic.
  • Exhibit a strong commitment to advocating for the rights of older adults.
  • Experience in managing a brokered budget efficiently, including tasks such as invoicing and financial processing.

Clinical Case Manager

  • The ideal candidate will have a background in Nursing, or a related field with min 2 years’ experience in clinical care within home, community, or aged care settings. Desirable,12 months experience within STRC or HCP delivery.
  • Advanced skills in case management, assessment and clinical care planning, as well as working knowledge of wellbeing and reablement.
  • Ability to evaluate service provision to ensure high-quality care that meets standards.
  • High level of sensitivity, understanding of issues impacting older adults, and commitment to advocacy.
  • Strong understanding of Aged Care Quality Standards, Open Disclosure, The Aged Care Code of Conduct, and SIRS.
  • Proficiency in MS Office and Client Information Systems (Carelink+ preferred).
