Our Journey
Primary Care Support Team, as part of the Community Assessment and Response Directorate offers a variety of programs that assist individuals to managechronic health conditions, focusing on the assessment and delivery of client focused care. The team consists of Allied Health Assistants, Care Coordinators,Community Development Workers, Counsellors, Diabetes Nurse Educators, Dietitians, Exercise Physiologists, Psychologists, Occupational Therapists, Physiotherapists, Social Workers, and Podiatrists. The team is committed to working with communities, families and individuals who have poor health outcomesand to providing high quality, multidisciplinary programs which embrace client centred care, evidence based practice and the Social Model of Health.
We are currently seeking experienced and dedicated Grade 2 Occupational Therapists for Part-Time / full time Ongoing (Permanent) roles offering flexibleworking arrangements with total 1.8 EFT

A Day in the Life
As a Grade 2 Occupational Therapist, you will use your specialist knowledge and skills to conduct client assessment and intervention independently, incorporatingremedial, compensatory, educational and preventative strategies to achieve optimal evidence-based consumer care and to support consumer health andwellbeing through occupation and consideration of the person and their environment.
You will also provide supervision to junior occupational therapists and assist with the development of strong, evidence based practice within Primary Care SupportProgram and as well as the broader Community Assessment Response Team (CART).

About You
What you will bring and what you will need:
Dedication to safe best practice & quality care
Committed to delivering patient-centred care & patient advocacy
Enthusiastic, compassionate & able to work collaboratively
Bachelor of Occupational Therapy or relevant equivalent degree
Current AHPRA registration
Current Driver’s License
