As we continue to grow, we have a new opportunity within the broader Practice, Risk & Quality team, for a Case Management Practice Lead to partner with our Home and Community business leaders and their teams to co-design and implement a new case management approach in line with our new relationship centred Model of Care, Inspirit
Reporting to the Program Manager, Model of Care, you will establish and evaluate current case management process, procedure and practices and identify opportunities to redesign and lead practice change in support of contemporary case management that advocates for customer choice, voice and control. Supporting a consultative approach to enable choice and holistic wellbeing.

Additionally, your ability to manage and influence positive change with a strong commitment to delivering high-quality results and driving process improvement will be critical.
Your role will include;
- Establish and evaluate current case management process, procedure and practices
- Lead cross-functional collaboration with SME’s to assess current case management practices, identify gaps and opportunities for improvement
- Develop action plans to achieve project objectives
- Designing processes that align with internal client management and Tech systems
- Testing and documenting processes, workflows, protocols that promote seamless coordination, communication, and collaboration among Care Advisors, Community Workers, clients, families, and external service providers
- Researching and recommending better practices for emerging concepts in case management
- Monitoring the effectiveness of services to ensure alignment with the Model of Care
- Designing and implementing training programs to educate employees
- Facilitating training and coaching to support cultural change
- Travel to sites across Sydney Metro, Regional NSW
The successful candidate will be highly organised, with the technical capability to lead practice change and a genuine passion for customer choice and service improvement. In addition to;
- Tertiary education in social work/human services - preferred, other relevant fields (e.g., nursing, psychology, and other allied health) will be considered
- 5+ yrs. experience in a leadership role with demonstrated expertise in practice development, quality improvement, and staff supervision in aged care/community sector
- Strong understanding of evidence-based practices, regulatory requirements, and ACQSC standards
- Advanced people consultation, coaching and influencing skills
- Proven ability to lead and manage change, foster innovation, and drive continuous improvement initiatives
- Proficiency in project management, data analysis, and technology applications relevant to practice improvement initiatives
- Effective problem-solving and decision-making abilities, with a focus on achieving results and optimising resources
- Ability to work collaboratively in a multidisciplinary team environment and engage effectively with diverse stakeholders
