About the Role: The overall purpose of the Practitioner - Sustaining Tenancies is to provide person centred early intervention/prevention support to individuals and families at risk of homelessness. The Practitioner, will deliver essential services in providing prevention and post crisis support to clients to maintain their housing for the duration of need. They will work with the individual/family through ongoing assessment to determine the clients changing needs and provide ongoing support at helping them to sustain their tenancy, maximise their capacity to be independent, self-reliant and connected to community supports.
