A unique opportunity has become available for a Behaviour Support Practitioner located in the Leongatha South area to join our professional and friendly team.

Thanks to our commitment to service excellence, demand for our services has increased and we have the need to find a new practitioner in the Leongatha South area.  We will consider an entry level practitioner or someone with a sound knowledge of the Positive Behaviour Support Model of reducing the use of Restrictive Practices, as well as a strong commitment to delivering evidence-based, functional behavioural outcomes.

 

As a Behaviour Support Practitioner you will...

  • Support our clients (NDIS participants), along with their families and/or carers, to identify their needs through assessment and person-centred planning
  • Identify meaningful goals and practical strategies utilising The Positive Behaviour Support Capability Framework, with a commitment to eliminate restrictive practices, in the home and community
  • Develop, deliver and evaluate evidence-based support plans within a complex support space
  • Ensure individual plans reflect our clients' goals, passions and strengths
  • Build the knowledge, capacity and strengths of those who support our clients - family, carers, friends, etc

About you

  • Current suitability with the NDIS Commission as a Behaviour Support Practitioner AND/OR considerable experience providing support to people with behaviours of concern
  • Demonstrated ability to work positively in a multidisciplinary team
  • Outstanding communication, influencing and interpersonal skills
  • Confident computer skills
  • Sound writing skills
  • Current drivers licence and own reliable vehicle
